I am winding down my work here, trying to finish all the projects I have started, as well as see my full case load of patients each day. Today we had a couple really dificult emergencies in the clinic, a woman who had an ultrasound and was found to have triplets, one of which was in breech position. I don't know the statistics but I figure triplets are rare here. The most dificult for me was a 12 year old girl who came with her mother, who was insisting the girl had a vaginal infection. Many women here think they have [an] infection, but often they just have normal dishcharge or menstrual cramping. This girl, after being tested, revealed that she has Trichomonas, a sexually transmitted infection. Benit and I met in private to discuss how we should deal with the situation. We asked the mother to go out of the room to protect the girl's privacy. She admitted she had consented to sex with an older boy, she had wanted to try it, but was afraid her mother would find out, and was afraid after, and didn't want to do it again. We told her we would protect her privacy and give her treatment, but she had to know and understand the ramifications of having sex at such a young age. She listened intently, cried a bit, climbed into my lap like the little girl that she is, and threw her arms around my neck, so assured and worried and appreciative. Poor little thing. I hope to really ramp up the education programs in this area for girls, teach sex- ed, womens health, and much more.
Everyone is out in the streets watching the America's soccer games, every now and again I hear a huge collective cheer as someone scores a goal. We tried to go out for a cold beer but everyone is sold out because the town is out in the streets drinking all the beer and crowded around their little televisions in the street. It's a great community spirit. I am pretty satisfied, getting a lot done here. I finished the preantal care card, as well as the dossiers (charts) for all the prenatal visits. We have a medicine inventory finished, as well as a list of the meds we use the most. We have an idea of when we will run out of certain meds in the near future. We have several new medicine distibutors and more ideas of how to buy meds and supplies. I am almost done with the prenatal protocols and have followed up on several at risk kids in this community. I am almost done here for now. Gale has offered me a position, along with 2 or 3 other nurses, as interim medical managers to fill in each month throughout the year, acting as a group coordinator. i hope i can work this out! I am honored and pleased to be offered such a place on the PID team. I look forward to so much!
